What is a Patient Centered Medical Home?

A Patient-Centered Medical Home is a team-based model of care led by the primary care physician, PCP who provides continuous and coordinated care throughout a patient’s lifetime to maximize health outcomes. At NEW Health, our medical home team typically consists of your physician, PCP and nurse, as well as other providers as needed such as a behavioral health specialist, health educator, or nutritionist.
Your New Health primary care team is responsible for providing you with care that meets your specific health care needs; and appropriately arranging care with other qualified professionals when needed. This includes the provision of preventive health services, treatment of acute and chronic illness, and assistance with overcoming barriers to good health care. Our team of health professionals, coordinated by your personal physician, works collaboratively with you and your family to provide enhanced access to health care, improved communication, care coordination and integration with other health professionals. NEW Health’s PCMH model puts patients at the center of their care and involves patients in setting their own health care goals with an aim toward wellness and healthy living.
